TWO SEPARATE REGISTERS IN ONE FORM
There are two separate registers. One for people who have experienced adverse reactions after a Covid vaccine.
The second is for people who have experienced duress resulting in them having a vaccine they did not want, or a sanction when they have declined a vaccine.
